Please read the following information carefully regarding the nature of our services and payment structure.
Concierge Nursing Services Are Private Pay & Elective
We do not accept Medicaid or Medicare. Services offered are non-medical support, education, care coordination, and advocacy — and are not a substitute for medical or Medicaid-covered care.
No Medicaid Participation
Eldercare Concierge & Consulting Services has not enrolled as a Medicaid provider, Medicare provider, or any other federal or state health care program provider.
The services provided under any service agreement are private pay only and will not be billed to Medicaid, Medicare, or any government-funded health program.
This notice is included in all service agreements as a required acknowledgement. The client acknowledges and agrees that services are private pay and will not be submitted to any government health program for reimbursement.
Medicaid Safe Attestation
"I acknowledge that I am choosing to receive private-pay concierge nursing services that are not covered by Medicaid, and that I am paying voluntarily for convenience and support services only."
This attestation is required as part of the service agreement and must be initialed or acknowledged by the client prior to the commencement of services.
Voluntary & Elective Services Statement
Participation in this concierge nursing and consulting service is entirely voluntary, elective, client-requested, and provided for convenience, education, advocacy, and support only.
No services are required as a condition of receiving Medicaid or other benefits. Clients retain full access to all Medicaid, Medicare, and government-funded benefits regardless of whether they choose to engage our private-pay services.
Client Right to Medicaid Disclosure
Medicaid services are available through enrolled providers. Choosing to receive private-pay concierge nursing services does not affect your eligibility for Medicaid benefits. You retain full rights to access Medicaid services through enrolled providers at any time.
Client Rights Acknowledgement

Service Agreement Required
All clients are required to sign a service agreement prior to receiving services. The agreement includes the full Medicaid non-participation clause, the client attestation, the voluntary/elective services statement, and the client right to Medicaid disclosure outlined on this page. No government program billing will be initiated by the provider under any circumstances.
